  • Before you turn in your form, you will need to enroll in 4-HOnline.
  • Prior to a 4-H member participating in a 4-H event, they are required to submit an academic eligibility form. 4-H follows the same “No Pass, No Play” rule as sports and other extra-curricular activities. This form is requested in accordance with the requirement of the Texas Education Code and in cooperation with the Texas Education Agency and local school board policies.  When completing this form, you have two options: 1) Parents/Guardians complete the firsts section and submit the eligibility form to the County AgriLife Office 2) The family completes the form, gets the school’s signature, and then submit it to our office.  If you go with Option 1, we will get the school to complete their section for you. It is totally up to whatever works best for you! If you have any questions about the eligibility form, please reach out to our office.
